Database Combo Box: Generate form select inputs from database queries

This is a simple class that can generate form select inputs from database queries.

It can connect to a given MySQL server and execute SQL SELECT query to retrieve the values of two given fields of a certain table that match a given condition.

The class can generate HTML for a form select input that presents options with values and titles taken from the two fields returned in the query results.
